Case Management

Every member can get a service facilitator for case management who is staff at Yahara House.  Members who prefer to maintain a relationship with an outside service facilitator are free to do that, and have a designated staff point person at Yahara House.  They manage CCS (Comprehensive Community Services) paperwork; help connecting to services in the community, including housing, food stamps, Medicaid and Medicare,  finances, and healthcare management;  accompany the member to appointments and refer or personally assist with job coaching.
